- About Our Practice
Rely-Able Care Services provides outpatient psychiatric evaluation, medication management, and psychotherapy to patients located in Florida. A provider-patient relationship begins only after an initial evaluation and signed consent, not by visiting our website or sending an email.
- Appointments, Cancellations & Billing
- Scheduling: By appointment only. Please arrive 10 minutes early for check-in or log in early for telehealth.
- Cancellations: We require 48-hour notice. Late cancellations and no-shows may be charged the full session fee.
- Payment: Co-pays and self-pay fees are due at time of service. We submit claims to contracted insurers, but you are responsible for benefits verification.
- Records requests: Copies are provided upon written request per Florida law §456.057. We charge only actual copying costs.
- Privacy Practices – HIPAA & Florida Law
We protect your Protected Health Information (PHI) under HIPAA and Florida Statutes.
Information we collect:
- Personal: name, date of birth, phone, email, address, insurance ID
- Health: psychiatric history, treatment notes, medications, labs
- Technical: IP address, browser type, portal usage, and mobile number for messaging
How we use it:
- Provide treatment, coordinate care, and document services
- Schedule appointments and send healthcare communications
- Bill insurance and process payments
- Meet legal and quality requirements
Your rights:
- Request access to and copies of your records. For psychiatric records, we may provide a summary first; complete copies are sent directly to a subsequent treating psychiatrist upon your written request
- Request amendments, restrictions, and an accounting of disclosures
- Request confidential communications
- Receive breach notification under Florida law within required timeframes
Confidentiality limits: We maintain confidentiality except when required by law, imminent risk of harm to self or others, suspected abuse of a child or vulnerable adult, court order, or for treatment, payment, and healthcare operations.
Security: We use encrypted EHR, HIPAAcompliant telehealth, and staff training. All employees are trained to protect medical records.
- Telehealth Services – Florida
Telehealth is provided under Florida Statute §456.47.
- You must be physically located in Florida at the time of the visit
- Care is provided via secure video, not email or text
- We practice to the same standard as in-person care and document all services
- Risks include technology failure and limited physical exam; benefits include access and continuity
- Controlled substances for psychiatric disorders may be prescribed via telehealth when clinically appropriate and following federal and state law
- You may decline telehealth at any time
- Communication Consent – Email, Phone & Text Messaging (A2P)
We use text messaging only for healthcare operations, not marketing.
What we text: Appointment reminders, confirmations, rescheduling, intake forms, telehealth links, billing notices, and brief care coordination. We do not text therapy content or diagnosis details.
How you opt in (consent is required):
- Checking “Consent to Text” on intake paperwork
- Giving your mobile number verbally at scheduling and confirming consent
- Confirming via patient portal
- Texting us after staff invitation
Consent is not a condition of treatment. We keep a record of your consent.

Opt-out and help:
- Reply STOP anytime to unsubscribe. You will get one confirmation text.
- Reply START to re-enroll.

Important SMS terms:
- Message frequency varies by care, typically 1 to 4 per month
- Message and data rates may apply per your carrier
- SMS is not secure or encrypted. Do not send PHI, crisis information, or emergencies by text
- Mobile information will not be shared, sold, or rented to third parties or affiliates for marketing or promotional purposes. We share your number only with our HIPAA-compliant messaging vendor as needed to deliver messages
- For email, standard security applies. We do not discuss sensitive clinical matters by email unless you request it in writing
- Medical Records Retention
We retain records for at least five years from your last patient contact, per Florida Board of Medicine Rule 64B8-10.002. For minors, we retain until at least two years after age 18. If the practice closes or relocates, we will provide notice as required by §456.057 and offer you a copy of your record.

- We may use secure third-party platforms for scheduling, telehealth, and payments. These vendors maintain their own privacy standards under business associate agreements
- Accessibility
We are committed to accessibility under the Americans with Disabilities Act and WCAG 2.1 Level AA. We strive for readable fonts, sufficient contrast, keyboard navigation, and screen-reader compatibility. - Emergencies
We do not provide 24-hour crisis care via text or email. For psychiatric or medical emergencies, call 911, go to the nearest emergency department, or call/text 988 Suicide and Crisis Lifeline.
- Changes to This Policy
We may update this policy to reflect legal or carrier requirements. The effective date at the top indicates the latest version. Continued use of our services after posting constitutes acceptance.
